$100 per month, per enrolled person

Each enrolled person receives three 20-minute chiropractic sessions per month. The $100 monthly fee applies to one person only and does not cover the entire family. Each additional family member who wants to receive membership visits must enroll separately at $100 per month.

Family members who are not enrolled may receive care at the standard follow-up rate of $65 per person, per visit.

All services are private pay and not reimbursable by insurance. As a Medicare provider, Back Foundation Chiropractic submits qualified claims directly to Medicare.
