Corporate Wellness Service Agreement
This Corporate Wellness Agreement ("Agreement") is entered into between Back Foundation Chiropractic Abrahams, PC ("Provider") and the participating business entity ("Client") for the provision of on-site chiropractic and wellness services to Client's employees.
1. Scope of Services: Provider will deliver on-site chiropractic and wellness services, which may include chiropractic adjustments, assisted stretching, therapeutic massage, and red-light therapy. Services are preventive and supportive in nature, not diagnostic or curative.
2. Session Structure: Services will be provided for up to five (5) employees per month. All sessions will be scheduled and delivered on a single designated day each month, coordinated in advance between Provider and Client.
3. Term and Payment: This Agreement is valid for a period of twelve (12) months and shall automatically renew unless terminated in writing by either party with 30 days’ notice prior to the renewal date. Client agrees to automatic monthly payments of $500 due on the first day of each month.
4. Privacy and Confidentiality: Provider agrees to maintain the privacy and confidentiality of all employee health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). No personal or medical information will be shared with Client or any third parties without the employee’s express written consent.
5. Limitation of Liability: Provider shall not be held liable for any outcomes or injuries that occur outside the scope of care provided during scheduled sessions. Employees are encouraged to consult with their primary care provider regarding their individual health conditions.
6. Cancellation and Rescheduling: Client may cancel or reschedule a monthly session with at least 48 hours' notice. Missed sessions are non-refundable but may be rescheduled at the Provider's discretion.
IN WITNESS WHEREOF, the parties have executed this Agreement with initial payment.
